Printed out onto white card and cut out all the elements. Using a 5 inch square card with the corners cut off, I attached the base picture with d/s tape and then added the remaining pieces with foam pads.
I attached the two tags back to back with d/s tape, punched a hole and added matching embroidery thread to finish.
