I reduced the size of the sheet to fit a 5 1/2" card, then printed 2 copies on a sheet of 8 1/2 x 11 matte photo paper and one more copy on another sheet, along with cup874566_70864, which I used as an insert (also considerably reduced in size).

Attached the topper and insert with dst, then cut the inner white square of the second topper and attached with thin foam pads. Added decoupage pieces and a sentiment cut from the third copy of the topper (with Pinflair Glue.) Such a cute design!
