I printed and cut out this file and stuck the base card on to card stock. I then assembled the decoupage on to the card.

I printed the sheet on quality cardstock and cut out the design. I used foam pads to decoupage and double sided tape for the rest of the card construction.
I adhered two red rhinestones to the card front before matting it with red cardstock and attaching it to a blank, white card.
