I have printed motif on good quality 150g photo paper, cut all the parts, rounded edges a bit and assembled with double-sided adhesive pads.
I have used a piece of background paper, designed by Jean King and on the empty tag I've put a sticker with the word Congratulations

Printed onto matte coated paper, then mounted onto white card. I used silicone for the decoupage and finished of with a blue insert.
