I printed and cut out this file and stuck the backing paper on to card stock. I then stuck one of the large rectangles on to the card with sticky tabs and then cut out the inner rectangle and stuck that on with sticky tabs.

I added a "With Sympathy" at the top right of the inner rectangle.
