Print onto good quality card. Using a craft knife cut along the horizontal line under the top middle 3 rectangles/squares and above the middle 3 large circles. Do the same under the 3 large circles. Fold the card as indicated by the arrows, remembering to fold the middle circles in the opposite direction to the top and bottom.
