Sheet reduced to A5 size, printed once onto A4 white card. In my graphics prog I added a name & age with wordart,& made a similar colour greetings tag to match image, then printed this onto smooth decoupage paper.

Cut out & folded main card, decoupaged the shirt & football with foam pads, added greetings.
