Its Your Special Day Easel Card Plus Envelope
This kit includes 5 pages plus instructions. The 5 pages include the Envelope front, envelope back, backing paper, card base, topper with decoupage layers. There are 3 labels. One label is blank and the other labels say Happy Birthday and Happy Mother's Day.

I printed on matte paper, printing twice for extra decoupage. Added the layers using foam pads and finished with gems and glitter. Also made up the matching envelope.
