I printed out front and back sheets onto heavyweight matt photo-paper, and the others onto medium weight matt photo-paper, cut out, folded where indicated and attached front and back to insert with wet glue, then added decoupage to front and back and finished with the year tag.

A great design, makes a lovely year long gift.
