I found this a bit confusing to get right (admittedly working through flu), so have some extra photos as well.

I printed the two toppers on CraftsUPrint paper, and assembled them with foam pads, using a blue pen to edge the larger two layers. (Both of these could easily make individual cards if you wanted to.)

I printed the main body of the card on CraftsUPrint paper, scored, cut, and folded. The straight central fold will be a valley fold when the card is "up". See separate photo for this folded but without the toppers in place. I attached the toppers to the folded halves with the marked valley folds folded, and the toppers side by side and lined up with each other and with the long central fold (flattened at this point). Again, extra photo shows this stage.

I attached the ribbon to the base with foam pads, keeping it well forward on the base so as to support the toppers without them falling backwards. The buttons at either end are also on foam pads: the sentiment is simply glued flat to the ribbon.

The main card is now complete.

I printed the two sheets of the envelope on ordinary paper. Scored, cut, assembled with a tape dispenser. Be careful to assemble with as much room as possible, as the envelope is a very exact fit, especially if foam pads have been used.

This is a really impressive card (or cards!), and once you've figured out what goes where, very easy to make up.
