I printed the design onto good quality photographic paper and cut it out. I scored and folded the card as per instructions. I assembled the decoupage using thin foam tape. I cut out the aperture on the card as directed. I assembled the box using double sided tape.
I attached one of the tags to the card with a guitar brad from my craft box.

I printed the design on card stock and cut out the elements. I scored and folded the lines as per the instructions. I used soft pads to assemble the decoupage. I cut out the aperture in the card as instructed. I assembled the box card using glue.
