I printed off the sheet on good quality card as the Ballerina has to stand upright. Cut out all pieces and folded along the solid lines. I then cut out the dotted lines with a craft knife. Attached 2d cut out pieces to 'Ballerina' and 'Happy Birthday'.
Finally I added foam pad to the full teddy ballerina and attached to card.
