Cut out all elements. fold/bend papers a little to give the natural curve of pages. Put foam pads or silicone on the short page edge sides only. Run some glue down the spine of the page. Glue in place. Take of foam pad backing and let them stick down also.

You need to do this for every page. Once complete you can either give this as you finished card or mount it as a topper. The rest is up to you just have fun.
