Printed out onto white paper, trimmed the edges and cut into two pieces. Using the matching card (cup45058_107) I placed it onto the main card under the topper.

Then using the 2nd piece, I cut out decorative corners, placed a circular piece of white card to the centre and then placed inside the back of the card to finish.
