I printed the sheet onto Super Smooth white card and trimmed off the white borders. I then used a score board to score at the relevant places to make a card/box combination, cutting off the edges of the card side. After cutting out a "window" and putting acetate behind I stuck down the edge to make the box. I took a panel of the card I'd cut off and punched both ends with a paper punch before simply decorating with some gold ribbon and then attaching to the front of the card.

A nice background to have in your digital craft library as it can be used for many styles of cards and crafting.
