I printed the card onto 220gsm card (the minimum weight I would recommend). With a craft knife I cut out the white segments around the design. I folded the card where indicated.

As I had some pink matching card available I used this to stick onto the inside back of the card but this is not necessary - in fact I think it would look better with a plain back.

Very, very simple to make but looks much prettier in reality than in the photo.
